Transitional Care
Safe Bridge Between Hospital and Home
Transitional care at CNR ensures patients discharged from hospitals continue to receive the medical support they need, reducing complications and preventing re-hospitalization.
Transitional care at CNR — bridging the gap between acute hospital care and home recovery with 24×7 specialist support and intensive rehabilitation.
What is Transitional Care?
Structured Recovery After Hospital Discharge
Transitional care is a structured post-hospital recovery program for patients who are medically stable but not yet ready to return home safely. It provides continuity of medical care, physiotherapy, nursing support, and specialist consultations in a supported inpatient environment.
CNR's transitional care facility in Dilsukhnagar is ideal for patients recovering from brain surgery, stroke, major trauma, dialysis, or any condition requiring medical monitoring after hospital discharge. Our 50-bed facility includes ICU and HDU-step-down units for high-dependency patients.
Unlike home care, CNR's transitional care provides immediate access to specialist doctors, emergency response capability, and comprehensive rehabilitation — all in one place.
